How to file and request a partial decision for Part E

How to obtain the evidence needed for Part E

How to request and obtain financial compensation from $125,000.00 to $250,000.00 depending on your claim.

Understanding  the medical card and what is needed to make  it work.

We will have  forms to fill in that will help you obtain the information needed to get your claim paid under Part E.

 

Thanks for coming to the last meeting for those who attended. For the ones that missed the meeting, this is what you need to do regarding the National Screening Test.( If you are not familiar with it, please let us know.)

    1. request a Heavy Metal test, blood and 24 hr urine;

    2. request a liver function test;

    3. request a kidney function test;

    4. request a beryllium test;

    5. request that the x-ray be read by a certified B-Reader; and

    6. request a copy of all reports and results.
